Reports to the Nursing Director or designee. Applies the nursing
process utilizing Relationship Based Care framework. Establishes
strong patient family relationships. Collaborates with other
professional, clinical and ancillary staff in providing quality
care to patients. Displays strong teamwork. Exhibits self-care and
self-knowing in order to be empathetic and compassionate with every

rounds with physicians. Clear, concise communication of patient
condition using nursing tools during all hand offs including
bedside report. Participates in shift briefs, debriefs, and huddles

supervises only those nursing measures which the nurse knows, or
should know, that another person is prepared, qualified, or
licensed to perform. Fosters a sense of trust and collaboration

Retains professional accountability for nursing care when
delegating nursing intervention. Patient Care Delivery: Under the
direction/supervision of a Registered Nurse Implements the nursing
plan of care. Implements strategies to provide for patient and
family participation in health, promotion, maintenance, and
restoration. Utilizes critical thinking skills and employs the use
of the teach-back method. Keeps patients/families informed and
answers call lights promptly. Initiate nursing actions to assist
the patient to maximize his/her health capabilities. Provide
nursing care without discrimination on the basis of diagnosis, age,
sex, race, creed, or color. Administers medications, monitors IV
and blood transfusions, monitors vital signs and performs routine
treatments and procedures. Performs phlebotomy as directed by the
needs of the department. Maintains patients records and charts,
recording pertinent information , medications, treatments
administered, treatment response and initiating patient incident
reports as needed Reinforces patient teaching and discharge
instructions. Resource Driven Practice: Seeks education resources
and create learning experiences to enhance and maintain current
knowledge and skills for his/her continuing competence in nursing
practice and individual growth. Including continuing education,
council and committee participation, and process improvement
activities. Seek education and supervision as necessary when
implementing nursing practice techniques. Demonstrates prompt shift
arrival and meets all attendance standards. Outcomes: In
conjunction with the Registered Nurse, evaluates patient
achievement of goals and when necessary, alters and establishes
strategies to meet revisions in the patient's plan of care.
Conducts purposeful rounding to assess, evaluate, and anticipate
needs. Participates in the review and evaluation of the quality and
effectiveness of nursing care. Receives patient feedback via call
backs, leader rounds, or surveys. Documentation is thorough and
follows core measure guidelines. Completes other job related duties
and projects as assigned. Performs other functions to maintain
personal competence and contribute to the overall effectiveness of

as assigned. ORGANIZATIONAL RESPONSIBILITIES Associate complies
with the following organizational requirements: Attends and
participates in department meetings and is accountable for all
information shared. Completes mandatory education, annual
competencies and department specific education within established
timeframes. Completes annual employee health requirements within
established timeframes. Maintains license/certification,
registration in good standing throughout fiscal year. Direct
patient care providers are required to maintain current BCLS (CPR)
and other certifications as required by position/department.
Consistently utilizes appropriate universal precautions, protective
equipment, and ergonomic techniques to protect patient and self.
Adheres to regulatory agency requirements, survey process and
compliance. Complies with established organization and department
policies. Available to work overtime in addition to working
additional or other shifts and schedules when required. Commitment

and Experience The knowledge, skills and abilities as indicated
below are normally acquired through the successful completion of an
LPN accredited school of nursing Must have a current license to
practice as Licensed Practical Nurse in the State of Indiana. A
minimum of 1 to 2 years of job-related experience is preferred.
Knowledge & Skills Demonstrates a high level of knowledge and
competency while participating in patient interdisciplinary
planning as well as performing nursing care procedures,
assessments, treatment and techniques under the supervision of a
Registered Nurse. BLS within 90 days of obtaining position.
Requires strong analytical and organizational skills necessary to
participate in the implementation of plans of patient
care/education in changing patient situations with recognition of
actual and potential problems, patient/family involvement and the
collection of data related to unit nursing care concerns.
Demonstrates strong interpersonal skills necessary to establish and
maintain positive therapeutic relationships with patients and
families and effective working relationships with staff, physicians
and others. Requires team member skills in order to provide
constructive feedback to peers regarding nursing issues, and
participates in unit and hospital based decision making.
Demonstrates flexibility and ability to adapt and support change.
Demonstrates ability to communicate, both verbally and in writing,
in a clear and concise manner. Requires general working knowledge
of the normal growth, development, related behaviors, needs,
comfort, and range of treatments appropriate to the age (including,
adolescent, adult, and geriatric) and diagnosis of patient served
to support the provision of an individualized interdisciplinary
plan of care. Requires an understanding of the need to be
responsive and reasonably adaptable for scheduling purposes, when
necessary, due to extenuating circumstances in support of
co-workers and department functions. Requires the ability to work
under the supervision of a Registered Nurse; takes initiative and
demonstrates the ability to be self-motivated. Demonstrates basic
computer knowledge and skills. Functions within the legal
boundaries of Indiana Nurse Practice Act based on the knowledge of
statutes and rules governing nursing. Working Conditions Works in a
patient care environment requiring physical exertion, frequent
changes in job demands, and certain undesirable patient care
activities. May be exposed to bio-hazards and blood borne
