Clinical Generalist Pharmacist; DH – Fort Wayne, IN Our client, a large healthcare entity in Northwest Indiana, has a need for a Clinical Generalist Pharmacist in Fort Wayne, IN. The clinical generalist will provide pharmaceutical care services to the patients of 2 hospital locations in Central and North Fort Wayne. Clinical generalists rotate through clinical and distribution services, however, anticipate being assigned to a clinical pharmacist role >60% of the month. Main Responsibilities: Multidisciplinary rounds 7 days per week Oversight, monitoring, preparation, dispensing, and proper documentation of patient medication therapy Advising physicians on matters pertaining to drug usage and control Providing drug information, patient information, and oversight of information and dispensing processes and systems is also critical. Clinical practices include: dosing and monitoring anticoagulants, antimicrobials including vancomycin, total parenteral nutrition, patient-controlled analgesia, and others. Generalists have ample opportunity to mentor pharmacy IPPE/APPE students, five (5) PGY1 pharmacy residents and one (1) PGY2 ambulatory care pharmacy resident.
