Job Description:
Job Title: LPN – Continuing Care Location: Grand Rapids, MI –
49503 Initial Duration – 3 months (High possibilities of Extension)
Shift– Days & Nights (7 AM - 7 PM/ 7 PM - 7 AM) Note: · You can
refer your friends or colleagues for this role or any other RN
role, we do offer a referral bonus of $750. · Stipend is also
provided to the candidate living more than 50 miles from the
facility · Other locations available in NJ. · Local/Travel
Contracts available in multiple departments in all states. Job
Summary: · Provides direct patient care under the direction of the
RN/Mid-level Provider/Physician, consistent with policies and
procedures, and unit-based routines. · Informs patients and
families about treatments and medications being received, equipment
being utilized, and other related activities as appropriate, in
coordination with the RN/Mid-level Provider/Physician. · Greets and
communicates with patients and families in a manner consistent with
caring and respect. · Communicates and collaborates with other
members of the health care team in order to ensure continuity of
care and coordination of service. · Documents patient chargeable
items according to standards. · Actively participates in safety
initiatives and risk mitigating measures where appropriate and
completes all position and unit safety related competencies and
requirements on a timely basis. · Recognizes appropriate and
ethical behaviours and identifies resources for resolution as
needed.
