Reimbursement Cutting Edge Technology The Licensed
Practical/Vocational Nurse (LPN/LVN) provides direct patient care
under the supervision of a Registered Nurse (RN) . In this role,
you will follow individualized care plans, provide treatments, and
collaborate closely with the interdisciplinary team to achieve
positive patient outcomes, with a focus on end-of-life care . What
You'll Do Deliver direct patient care in accordance with the State
Nurse Practice Act , including specialized end-of-life care duties.
Implement current nursing practices based on comprehensive patient
assessments and established care plans. Maintain accurate, timely,
and thorough documentation of patient care to reflect adherence to
the plan of care. Coordinate hospice services and communicate
patient needs effectively to the interdisciplinary team, including
hospice aides, occupational/physical/speech therapists, social
workers, dieticians, and clinical supervisors. Provide appropriate
pain and symptom management , evaluating patient responses to
treatments and medications. Ensure clinical documentation reflects
ongoing communication and coordination with physicians, facility
staff, and team members . Required Qualifications Graduate of an
accredited practical nurse or vocational nursing program Currently
licensed as an LPN/LVN in the State Minimum one (1) year experience
as a nurse in a clinical care setting Desired Qualifications
Hospice or home health environment preferred Location Office: 770
Kenmoor Ave, Suite 100, Grand Rapids, MI 49546 Service Area:
Wyoming, Caledonia, Lowell, and surrounding communities Schedule
Regular Hours: 8:00 AM–5:00 PM, Monday through Friday Weekend
Rotation: Required on rotating basis Pay Range $24—$28 USD
