flexibility. Job Summary: We are seeking a skilled and
compassionate Home Health Visiting Nurse to provide in-home care to
patients across Chicagoland Area. The ideal candidate is clinically
strong, reliable, and able to work independently while maintaining
clear communication with our interdisciplinary team.
Responsibilities: Conduct home visits to assess patient conditions
and deliver skilled nursing care Implement and evaluate
individualized care plans Provide wound care, medication
administration, IV therapy, and chronic disease management Educate
patients and caregivers on care instructions, safety, and wellness
Document care visits and updates using our electronic health record
system Coordinate care with physicians, therapists, and other care
providers Comply with all state and federal regulations, including
HIPAA and infection control protocols Requirements: Active RN or
LPN license in the state of Illinois. Minimum [1-2] years of
clinical nursing experience (home health preferred) Current CPR
certification Valid driver’s license and reliable transportation
Strong assessment and critical thinking skills Ability to work
independently and manage time effectively Familiarity with
electronic charting (e.g., Kinnser, Axxess, etc.) is a plus What We
