Job Description Job Description We are seeking Nurse
Practitioners/Physician Assistants with Full Practice Authority to
perform in-home/telehealth health assessments for patients in the
Chicago area. This is a unique opportunity to work with patients
who may be less engaged with traditional healthcare, helping them
better understand and access the care they need. What You’ll Do:
Conduct comprehensive health assessments in patient homes or via
telehealth. Review and update medical histories and current
medications. Document findings and care plans in EHR. Identify
health risks, chronic conditions, and gaps in care. Travel within
Chicago as needed (mileage reimbursed). What We Offer: Competitive
